Hours: The hours of this position may fluctuate based on identified and operational needs. ツTypically, the hours will be:Monday thu Fridayツfromツ8:00am to 4:00pm.ツ There may be some evening, weekend and/or on call duties; yet this will not be a regular schedule.

Job Requirements:

  • Certified Peer Support Specialist or Certified Recovery Specialist
  • Associate or Bachelor窶冱 Degree eligible in the Social/Behavioral field
  • Prior Case Management experience preferred
  • Excellent Verbal, non-verbal and written communication skills
  • Time management & organizational skill

Description:ツWe are currently seeking a full-time CPS / CRS for our Dunmore CTC facility. Hours are weekdays with various weekend and evening hours as needed.

Dunmore Comprehensive Treatment Center, a member of Acadia Healthcare, is a comprehensive outpatient substance abuse treatment facility serving patients diagnosed with the disease of addiction. The qualified candidate will work closely in a team environment with a team of clinicians, medical personnel, case managers and office personnel in accordance with State and Federal Regulations to provide care for our patients. They will be part of the Center of Excellence initiative under PA Department of Human Services. We offer our patients an out-patient environment conducive to recovery from the disease of addiction, specifically to the opioid class of drugs. Position Summary:ツUnder the supervision of the Case Management Supervisor, the CRS/CPS is ultimately responsible for providing community based outreach to both individuals and professionals, alike. The primary goal being to ensure we engage those in need of services, regardless of the service type, and effectively link them to said services; with follow up and wrap around care under the OUD 窶 COE designation. Additional responsibilities include:

  • Utilizing knowledge of a variety of care models and theories to understand and plan intervention and outreach strategies for a variety of community based providers
  • Development of network plans and outreach schematics
  • Monitoring contact progress and follow up, making changes to care plan(s) as need presents. Utilizing team approaches and evidenced based interventions based on responses provided
  • Working collaboratively with the a full spectrum Multi-Disciplinary Team to develop a comprehensive care/intervention plan that addresses priority needs, desired outcomes, an agreed upon plan of action, and reassessment;
  • Maintaining patient records related to screening, intake, treatment, continuing care, progress toward goals and objectives, discharge summary, and treatment outcomes that are accurate, concise, timely, and useful for coordinating treatment team planning, monitoring, and review.
  • Coordinating and cooperating with local agencies and organizations necessary to expedite unique individual case management plans and follow up.
