Title:ツHousing Case Manager/Program Assistant

Qualifications:ツAssociates degree in the Human Service field with two years relevant experience required; Bachelor窶冱 degree in the Human Service field preferred; Certified Recovery Specialist certification preferred. Must have a valid PA Driver窶冱 License and ability to drive and travel.

Description:ツHousing Case Manager/Program Assistant will provide case management services to individuals and families with a drug, alcohol and/or mental health disability residing in Berks Counseling Center窶冱 various housing programs. This may involve transitional, permanent supportive, or re-entry housing. This position also provides clerical support for the housing program.

Essential Job Functions

1. Accept referrals for the houses and interview potential tenants or guests.

2. Complete intakes on new clients when needed and initiate all pertinent paperwork, including a review of client窶冱 rights and consent forms.

3. Enter all initial client data in the computer through the HMIS system as well as updates and exits.

4. Maintain list of due dates for annual HMIS updates.

5. Maintain list of PSH applications.

6. Maintain client files, inclusive of opening and closing, and complete follow ups.

7. Receive clients and answer telephone calls and direct them to appropriate extensions.

8. Provide the orientation of clients to the house.

9. Maintain updated information on community services and resources.

10. Establish and maintain the client窶冱 case management files.

11. Assess the medical, social, vocational, educational, and general treatment needs of the client in light of preparing a written service plan in agreement with the client窶冱 goals.

12. Determine rent liability, monitor payments and assist with securing entitlement income for purposes of food medical and other needs.

13. Be responsible for communicating special needs to clients to their primary counselor.

14. Oversee the functioning of the houses.

15. Provide all the Case management needs of the clients to include: facilitating the access to needed community services, individual and group sessions to foster the development of various life skills; and general guidance towards the goal of maintaining recovery.

16. Complete monthly reports and maintain required statistical data.

17. Coordinate with house manager and ensure that the agency policies and procedures are followed.

18. Attend required trainings.

19. Perform other related activities and duties as requested.
