Qualifications:

  • High School Diploma, GED, Associates, or Bachelor窶冱 degree in Social Work, Counseling or related Human Service field from an accredited college or university
  • 1-2 years of experience working in behavioral health or addiction recovery setting preferred.
  • Previous experience in case management and/or utilization review preferred

Licenses/Certifications:ツCertified Recovery Specialist or Certified Peer Support certification required.

Position Characteristics:The CRS/CPS is a certified professional who manages the patient’s cases with a solid background of clinical knowledge and expertise and may be responsible for providing some therapeutic services to patients. Provides quality care through developing, implementing, managing and evaluating patient care plans. Collects clinical data, correlates this clinical data and provides clinical input to the treatment team. Conveys medical criteria and clinical information between the insurance provider and treatment team as warranted. May also correlate clinical data for business office as required.

Responsibilities:

  • Providing case management and care coordination services for patients in the clinic. Responsible for coordinating continuum of care activities for assigned patients and ensuring optimum utilization of resources, service delivery, and compliance with external agencies and referral sources requirements;
  • May also be responsible for conducting individual, and family educational sessions, various duties related to coordinating treatment and discharge activities, as needed;
  • Coordinates in a timely manner, issues or activities relevant to the team, and/or to the team窶冱 relationship within the facility or treatment milieu; and,
  • Perform other duties as assigned.
