Qualifications:

EDUCATION AND/OR EXPERIENCE:

  • High School Diploma required.
  • Minimum of 2 year窶冱 continuous recovery required
  • Three years of experience in community based outreach or demonstrated progressive advancement in responsibilities in healthcare setting such as clinic, hospital, etc. preferred.
  • Valid PA Driver窶冱 License required
  • Current CRS licensure, or eligibility to qualify and take the CRS educational course

Description:ツThe Certified Recovery Specialist (CRS) is an instrumental member of a multidisciplinary team working to address the disease of addiction within the Geisinger Medication Assisted Treatment Program and related addition treatment programs. The CRS will work within the community, providing outreach to patients with complex addiction, behavioral and psychosocial needs. The CRS acts as a resource to assist patients in navigating the health care system, and motivating them to engage and sustain engagement with the behavioral and addiction treatment system. Under the direction of psychiatry and addiction medicine leadership, the CRS will support patients, as a peer, along the road to recovery from acute addiction.

MAJOR DUTIES AND RESPONSIBILITIES:

*1. Assists in the outreach and engagement of patients identified as needing substance abuse support and treatment.

*2. Conducts regularly scheduled meetings with patients and appropriately engages them to identify interests, strengths, goals, dreams and aspirations while offering encouragement and empowerment through shared experience to enhance strengths and capabilities for members.

*3. Engages patients by way of home visit, hospital visit and accompanying patients to Providers appointments as needed.

*4. Facilitates adherence to treatment plan by early identification and resolution of gaps in care. Supports patients in planning for and attending 12 Step Meetings, finding a Sponsor and doing Step Work when appropriate.

*5. Communicates all concerns to clinical team and manager as necessary, seeking support and guidance proactively.

*6. Participates in multidisciplinary treatment team conference to maximize the treatment plan. Provides feedback to the treatment team regarding members and offering a unique perspective.

*7. Provides reinforcement of the treatment plan under the direction of the Addiction Coordinators.

8. Appointment scheduling and clerical duties as needed.

9. Builds relationships within the community to identify resources that are available to the patients i.e.: D&A providers, Single County Authorities, Emergency Services, Narcan supplies etc.

11. Promotes community integration through the connection of resources, linking to supports, mutual-help groups, social clubs, volunteer and job opportunities.

10. Performs all other duties as assigned by program manager or designee.

  • Demonstrates ability to use basic office equipment, computer, fax machine, copier, calculator, etc.

WORKING CONDITIONS/PHYSICAL DEMANDS:

  • Varies settings including but not limited to office, patients home, and hospital.
  • Travel required, personal vehicle with mileage reimbursement
  • Day shift with some evening hours required
