Qualifications:ツMinimum High School Diploma or GED. Some post-high school education, human service work, or heatlh care experience preferred. Certified Recovery Specialist or working toward CRS a plus.

Valid drivers license and reliable transportation.

Must be reliable, dependable, and able to work independently.

Able to maintain positive working relationships with EMS, medical staff, nursing staff, and community providers.

Experience and/or skill in managing crisis situations and working with potentially agitated persons.

Good writing, telephone, and communication skills.

Description:ツOn-call services and response to overdose event. Care Navigator will meet with individuals/families at local emergency room to provide screening, brief intervention, and referral to treatment services using a “warm hand-off” approach. Provide education and resource information to individuals/families. Works in collaboration with the healthcare team.

Responsibilities and Duties

Be available to respond to calls during scheduled hours. Shifts will be 5:00 PM to 8:00 AM during weeknights (Monday through Thursday). Weekend shifts will begin 5:00 PM Friday to provide 24-hour coverage until 8:00 AM Monday. Shift schedules will be divided between four Care Navigators.

Desired response is arrival at ER within 30-40 mins of receiving a call.

Meeting with individuals/familiies/heatlhcare team to provide services described.

Facilitate referrals to treatment, document all contacts, and report to project supervisor(s).

Attend scheduled training activities.

Maintain accurate records for reporting purposes.
