Qualifications:

Bachelor’s Degree, Master窶冱 Degree, or above from an accredited college with a major in medicine, chemical dependency, psychology, social work, counseling, nursing (with a specialty in nursing/health administration, nursing/counseling education or a clinical specialty in the human services), public administration, business management or other related field and minimum 2-5 years of experience in a human service agency, preferably in a drug and alcohol setting, which includes supervision of others, direct service and program planning.

Description:

Core Responsibilities:

  • 窶ェAssists agency staff members through individual and group conferences in analyzing case problems and in improving their diagnostic and helping skills
  • 窶ェReviews case records, evaluates performance of staff members, and recommends indicated action
  • 窶ェCarries a small caseload to counsel clients individually or in groups on planned or in emergencies
  • 窶ェWorks with Clinical staff to develop, coordinate, implement, and evaluate prevention and educational activities

General Responsibilities:

  • Begins, completes, and documents on the initial treatment plan, master treatment plan, integrated diagnostic summary and treatment plan reviews/updates.
  • Completes screenings and assessments of new patients
  • Clinician is responsible for completing the following
  • orientation note
  • initial treatment plan
  • psychosocial assessment or psychosocial update, and educational learning assessment
  • master treatment plan, progress notes and treatment plan review
  • Discharge or transfer summary and a discharge note
  • Develops individualized treatment plan with each patient
  • Performs individual therapy with patient/family
  • Attends staffing meetings
  • Shares information with team as obtained from patient窶冱 family, or others as indicated throughout program
  • Documents in the medical record on an ongoing basis
  • Provides education to patient/family on community resources
  • Assists patient/family with appropriate discharge plans
  • Acts as a liaison, providing referrals, between the patient/significant others, referral sources, community resources, & other social service agencies, as indicated.
  • Works with administration to enhance the outpatient treatment environment
  • Other duties as assigned by supervisor and/or CEO, COO, CFO or other management
  • Complete continued educational training which adheres with the requirements of the Department of Drug and Alcohol Programs
