Qualifications:ツMust have a valid driver窶冱 license, auto insurance and use of a vehicle during work and on-call hours; able to share office space and work as a part of a team; able to work evenings and weekends in meeting responsibilities.

Skills Required:ツMust have excellent verbal/written communication skills and work well with people; good, creative problem solving skills; excellent clinical skills including assessment, individual and group therapy; knowledge of behavioral health and criminal justice systems, community resources; ability to work independently, and be flexible/adaptive in handling changing priorities in a fast paced work environment; computer skills preferred.

Education and Experience:ツMust have graduated from High School and have full certification as an addictions counselor or a co-occurring disorders professional by a statewide certification body which is a member of a National Certification Body or certified by another state government窶冱 certification board.(Certification includes: CAADC, ADC, CAAC, CCJP, CCDP, CCDPD). Advanced degree preferred with experience working with dual diagnosed individuals. (Agency works with each clinician to develop a training program to expand their expertise in evidence based practices.)

Experience working with people struggling with both active Substance Abuse and MH challenges and forensic history.

Description:ツCTT is mandated to provide service to those individuals within the mental health system who present the greatest severity of symptoms as evidenced by their multiple physical, psychiatric, and legal conditions, extensive use of services, and lack of follow through with treatment. This population meets the highest level of medical necessity as defined by the state Adult Environmental Matrix. The Team provides multidisciplinary clinical review and assessment of individuals who may be young adults, heavy users of service, or actively involved in the criminal justice system and may also have co-occurring behavioral health diagnoses. The SA Specialist provides supports to all individuals assigned to his/her team as a part of the Community Support Program of Philadelphia. The SA Specialist completes Substance Abuse screener for all participants on the team and a full Substance Abuse Assessments as indicated with goals, objectives and intervention in the Treatment plan. The SA Specialist uses motivational interviewing and Integrated Dual Disordered Treatment approach to engage and work with team participants, using a harm reduction model for participants still active in their substance abuse. The SA Specialist will assist the individual in developing skills required to achieve sobriety and better manage their mental illness, enhance his/her comfort and abilities to function as a productive member of the community and will ensure that the mission, goals and philosophy of the CTT are operationalized within the team.

The SA Specialist works with individuals in their community environment assisting them in understanding, acquiring and maintaining independent living skills in the areas of: (a) their daily living situation; (b) interpersonal skills and social support/network building; (c) leisure and recreational support/skill development; (d) maintenance and enhancement of physical and mental needs, with particular focus on substance abuse issues; (e) obtainment of benefit entitlements and the skills to manage same; (f) vocational and educational needs; (f) accountability in fulfilling their legal obligations in order to remain in the community.

Essential Functions: Ability to sit for at least one hour at a time. Able to climb stairs when visiting consumers in their residence. Able to lift up to 20 pounds when assisting consumers with residential moves. (E) Able to work in community for a significant part of the day

Duties and Responsibilities:

  • Provides screenings and comprehensive assessments for assigned individuals with particular reference to substance abuse issues, including writing comprehensive reports.
  • Participates in daily multidisciplinary team meetings to review progress on cases in order to ensure communication and comprehensive approach to treatment, including collaborating with external teams to ensure coordination between systems; participates in SA Specialist staff group supervision as required for skills enhancement.
  • Provide input in case reviews to court personnel as deemed appropriate and necessary.
  • Provides individual and group treatment to address addictive behaviors that complicate primary MH issues, using Evidence Based practices and approaches such as: CBT, DBT, Trauma
  • Informed Care, Motivational Interviewing, IDDT and Harm Reduction.
  • Views individuals from a strengths approach in preparation of treatment plans, with a focus on recovery.
  • Monitors service delivery to individuals to ensure appropriate level of service given need as defined by Environmental Matrix score.
  • Establishes networks and liaisons with community service providers to resolve problems and overcome barriers to service, including attending linkage and treatment team meetings as needed; provides advocacy mechanism to ensure that individual needs are met.
  • Monitors individual窶冱 treatment and progress during hospitalizations by participation in in-patient treatment team and discharge planning meetings.
  • Maintains and updates community resource files, particularly around substance abuse resources but also behavioral, forensic and young adult population treatment resources, and provides information and shares with team and CTT community.
  • Completes all required reports and other paperwork in timely manner.
  • Participates in required clinical trainings and supervisions.
  • Participates in Clinical Care, Open Forum and other required agency meetings; attends in service training in order to meet mandated training hours.
  • Plans and assists individuals to increase community tenure, enhance quality of life, and attain highest level of independent functioning.
  • Provides after hours work including: a) participation in weekly on call rotation schedule to permit 24 hour/7 day a week access to service; b) and participation in evening and weekend shift work schedules required for delivery of services to CTT窶冱 caseload.
  • Completes all training require to maintain certification as a SA Specialist.
  • Performs other duties as required.
